New hire paperwork with HR will no longer include the ordering of a nameplate for the employee. Facilities Management will be the new contact for nameplate requests. Please follow the link above or you can find the form in the Facilities Resources section of the Employee Help Center (pictured below).
Please fill out all fields on the form and submit only once per person. Requests are automatically submitted on Fridays at 4pm and you should typically expect to receive the nameplate within 2 weeks of that submission.
